Lebanon OSHP Post investigating a five-vehicle fatal crash on I-75 in Warren County

LEBANON – The Lebanon Post of the Ohio State Highway Patrol is investigating a five-vehicle fatal crash. The crash occurred Aug. 4, at approximately 6:05 a.m., on Interstate 75 just north of mile marker 38 in Franklin, Warren County.

The preliminary investigation revealed five vehicles, a 2026 Chevrolet Trax, operated by Kelvin Munyurangabo, 25, Dayton; 2019 GMC Yukon, operated by Andre Foster, 52, Garden City, Mich.; 2017 Honda Civic, operated by Karen White, 59, Miamisburg; 2018 Ford Edge, operated by Samantha Dahl, 40, Centerville; and a 2010 Ford E-150, operated by Paul Young, 34, Dayton, were traveling south on Interstate 75 in the left center lane.

The Yukon and Trax stopped in the left center lane, the Trax behind the Yukon.

Munyurangabo exited the Trax. The Civic slowed and stopped behind the Trax. The Edge struck the Civic and then continued off the right side of the roadway. The Civic then struck the parked Trax. The Trax struck Munyurangabo and continued and struck the Yukon. The Civic rotated in the roadway, was struck by the E-150, and then struck the concrete median barrier. 

The E-150 continued and struck the Trax and the Yukon. Foster and his passenger, Tiffany Wilson, 55, Garden City, Mich., were transported by ground ambulance to Kettering Health Middletown with serious injuries.

Munyurangabo was transported by ground ambulance to Atrium Medical Center with serious life-threatening injuries. He was then transported to Miami Valley Hospital for further treatment.

White succumbed to fatal injuries at the scene of the crash.

Dahl was transported to Atrium Medical Center with serious injuries.

Young was not injured from the crash.

The crash remains under investigation by the Lebanon Post.

The Patrol was assisted at the scene by the Franklin Police Department, Franklin Township Fire Department & EMS, Joint Emergency Medical Services (JEMS), Clearcreek Township Fire Department & EMS, Lynx EMS, Ohio Department of Transportation (ODOT), Warren County Coroner’s Office, Warren County Prosecutor's Office, and Sandy’s Towing & Recovery.
