Highland County Healthcare Collaborative and Health Department request community participation in countywide health survey

The Highland County Healthcare Collaborative, in partnership with the Highland County Health Department, is inviting all residents to participate in the 2025 Highland County Community Health Survey. This important survey is a vital step in assessing the overall health needs, priorities, and challenges facing Highland County communities. 

The survey is open to all Highland County residents age 18 and older and is available online now. Responses are completely anonymous and will be used to guide community health planning, funding opportunities, and initiatives that improve local services and resources. 

“Our goal is to create a healthier Highland County, but we can’t do that without hearing directly from the people who live here,” said Jared Warner, Health Commissioner of the Highland County Health Department. “This survey gives everyone a voice in shaping the future of healthcare, mental health, substance use services, and more.” 

The Community Health Survey will ask questions about physical and mental health, access to care, nutrition, housing, transportation, and other social factors that affect health. The information collected will be used in the Highland County Community Health Assessment and will guide the development of a Community Health Improvement Plan. 

“This is your opportunity to be part of a community-wide effort to improve health outcomes,” said Warner. “Whether you're facing challenges or seeing improvements, we want to hear your experiences.” 

To take the survey, visit highlandcountyhealth.org and click the survey link, or scan the QR code below. It takes about 10-15 minutes to complete. Those who participate will also have the chance to enter a drawing for local gift cards and other prizes, as a thank-you for their time and input.
